NDIS - National Disability Insurance Scheme  

Centacare Southwest NSW is a registered National Disability Insurance Scheme provider. Centacare’s NDIS Officers can assist you with the implementation of your National Disability Insurance Scheme (NDIS) plan. Our NDIS Officers have knowledge and expertise in all plan areas, including Coordination of Supports, Plan Management and Delivery of Services.

Our NDIS Officers work with you to:

  • help you understand your NDIS plan
  • ensure your plan runs smoothly and efficiently
  • explore options to identify the services and service providers that best meet your individual needs, exercising full choice and control
  • provide support to connect with service providers as outlined in your NDIS plan
  • support you to access and navigate the NDIS Portal
  • establish and renew service agreements
  • address plan issues

Coordination of Supports

Our dedicated NDIS Officers assist you to find individualised, quality supports in order to achieve your goals and to participate more fully in the community.

This can include initial assistance with linking you with the right providers based on your needs, assistance to source providers and coordinating a range of supports.

Our NDIS Officers aim to build your capacity to manage of your own supports, strengthening your mental health recovery journey.


Plan Management

Our dedicated NDIS Officers provide plan management services that offer you the freedom to choose service providers (even non-registered providers) with the benefit of having someone support you with your financial planning for the life of your plan.

Our Finance Team run a smooth and efficient process to ensure timely payment of invoices.

The Team will also send out monthly statements to keep you up to date with your spending and the balance remaining of your NDIS funds.

Delivery of Service

Our dedicated NDIS Officers will help you to find support and information to access services from your plan; some of these may include Family Counselling, Family Education, and Psychological Services.

Family counselling can help support you in your relationships and assist with different issues in your life.

Family education can help with improving parent/child relationships from a range of courses and programs.

Psychological Service can provide assessment and treatment for participants with mental health problems to improve wellbeing.
